Twisted Squirm appears on the ceilings. Getting too close to him will cause him to reach down and grab the player. Clicking side to side quickly enough will let the player go, otherwise, Twisted Squirm damages them. After Twisted Squirm grabs a player, he traverses to a different area.
This ravenous Twisted hangs from the ceiling looking for books to eat. Look up, or you might just end up in this Twisted's grip. if caught, your best chance is to struggle free before it's too late.
