1. APPEARANCE

5 tells you can catch with the naked eye at the check-in window — no equipment needed. Several guarantee a low-pitched, distorted voice as a bonus confirmation.

7 tells only appear on CCTV — the patient looks normal at the window and in photos. Four cameras cover the hospital: check-in, lobby, medical, and emergency (the last unlocks on Shift 3).

Watch for broken cameras — fixing one is a hold-interact, but a Camera Figure can appear on the static and cost you 20 sanity.

MONSTER-CALMING TREATMENTS

Per the wiki, Eyedrops, Maple Syrup, Organ, and Transplant are the only treatments that can be given to monsters: Eyedrops temporarily stop the Mass of Eyes, and the other three make the Bed Monster leave. Eyedrops, IV Drops, or a held coffee also defuse death-ritual candles without sanity loss.

[ SANITY ]

Sanity — Your Health Bar

Per the wiki's Sanity page: you start at 61–65% (more with upgraded classes), reaching 0% kills you, and below 35% the music slows and the world desaturates. Job stress alone bleeds 1 point every 50 seconds — so sanity is a resource you spend and refill, not a bar you protect perfectly.

Shift Walkthrough — What Unlocks When

The first four shifts are tutorials, per the wiki's Shifts page. Three animal deaths fail the run.

SHIFT 1

Two scripted patients, never anomalies: Stomach Ache (needs Herbs) and Dried Eyes (needs Eyedrops). Ends with Dr. Harlow unlocking the shutters and security cameras. Always pays $63 if nobody dies.

SHIFT 2

Real randomized patients begin — anomalies can now appear, and story characters (Barney, Ron from Accounting, Ratthew) can show up and are always safe to admit. Ends with the Emergency Wing (Rooms 6–8) and the store unlocking.

SHIFT 3

Same gameplay loop. Ends with Dr. Harlow unlocking the taser and reminding you to check cameras and polaroids.

SHIFT 4

First Ambulance Event: after a news broadcast about an anomaly attack, six patients rush in at once — some on fire or critical, and anomalies often sneak in among them. Completing it adds bonus pay. An ambulance returns every 5 shifts after this with more critical patients.

SHIFT 6

Dr. Harlow's last appearance. He warns that three animal deaths fail the run, and reminds you to manage your sanity.

Events & Emergencies

Emergencies are timed — the countdown is visible through walls, and failing one kills the patient.

Ambulance Event

Recurring from Shift 4, then every 5 shifts. Six-plus patients flood in at once, some on fire or in critical condition — and anomalies frequently sneak in with them.

Response: Triage fast, photo-check what you can, and expect a mid-recovery monster. Bonus cash on completion.

Fire in Room

Any patient room can randomly catch fire (occupied or not). Standing near it costs 1–3 sanity per second. A new patient who walks into a burning room faints on contact — but if they enter the fire WITHOUT fainting, the wiki says it's a guaranteed Shapeshifter.

Response: Use the fire extinguisher — putting flames out by hand is slower and costs more sanity.

Patient on Fire

60 seconds

A burning patient runs in screaming. Extinguisher-doused patients are fully healed; hand-doused patients still need Ointment afterward (at the window or in a room). They can still be an anomaly.

Response: Extinguish, then treat with Ointment if you doused by hand.

Patient Fainted

60 seconds

Patients faint at check-in, from walking into fire, or from being tased/shot. You have 60 seconds to carry them to a bed.

Response: Carry to their room — or throw them in the trash can, which skips treatment entirely and does not count as a death.

Death Ritual

40 seconds

An occupied room dims; the patient levitates surrounded by ritual candles and the door takes longer to open. Removing candles bare-handed costs 1–3 sanity each.

Response: Defuse candles with Eyedrops, IV Drops, or coffee equipped (no sanity loss) — or tase the patient to break the ritual instantly.

Bed Monster Grab

30 seconds

If a room has the Bed Monster under the bed, it can grab the patient as they leave and drag them under.

Response: Give it Maple Syrup, an Organ, or a Transplant (equip the treatment and enter the red zone near the bed).

Monster Emerging (Tendril)

45 seconds

A patient recovering in Room 8 (surgery) can sprout tendrils. You must complete the surgery minigame to save them.

Response: Do the surgery — never tase or shoot the Tendril, which kills your patient automatically.

Critical Patient

60–120 seconds

During ambulance events, patients arrive in critical condition and must be brought into recovery before the timer ends.

Response: Get them into recovery — or tase/shoot to faint them and trash them, which eliminates the death timer.
